We've listed the legitimate, active campus surveys here to help you distinguish phishing from real surveys:

- Subject: Submitting Claims to Aflac Just Got Easier
- Summary: Aflac, one of UC’s benefit vendors, will send a bulk email to faculty and staff enrolled in one or more of UC’s Supplemental Health Plans (Accident Insurance, Critical Illness Insurance and Hospital Indemnity Insurance). The purpose of the email is to advise of enhanced functionality available on the Aflac participant web portal. The email addresses were voluntarily provided to Aflac by participants when they enrolled in the plans. Some are UC work email addresses and some are personal email addresses.

- A survey was sent out via email with this subject line from an outside market research firm, Consumer Evaluation & Insights.
- This survey is not UC affiliated and UC cannot make any assurances regarding the use, privacy, or security of any information provided.
